National Health Partners currently provides services across Ontario and can be accessed via our toll-free number for added convenience. The majority of services are provided on a fee-for-service basis allowing for a flexible approach to service delivery which best meets the client's needs.

Disability and Case Management Services:

At NHP, we believe our approach to Disability Management can be thought of as the people, materials and resources that are used to:
prevent disability
encourage rehabilitation and return to work
minimize the costs (financial, social, emotional) associated with disability for individuals and for the organizations in which they are employees/members

NHP provides disability management services in a client-centred manner, assisting individuals in their work re-entry process. Because communication is the cornerstone of effective vocational rehabilitation, we work diligently at developing close working relationships with our customers. We believe that this demands a local presence. NHP's services include assessment, opinion and intervention in cases involving physical, emotional, cognitive and vocational problems in order to maximize the individual's ability to function in all aspects of his/her life. Increasingly, disability is being viewed from a bio-psych-social perspective. In other words, it affects all domains of the individual's life. We are experienced in identifying the various factors that may prolong a disability and impede the individual in his/her return to work. Once we isolate the barriers to return to the labour force, we work with the various stakeholders to overcome them.

Our disability management consultants are highly qualified. They must:
be graduates of a university level program related to health and counselling
be members of a regulated health profession OR
have RRP and/or CCRC designation with Canadian Association of Rehabilitation Professionals

Rehabilitation Therapy Services:

NHP's rehabilitation services are provided by regulated health professionals who provide assessment services for a variety of orthopaedic, neurological and functional problems. The professional team includes physiotherapists and occupational therapists who are specifically trained in assessing function as it relates to the work environment and/or activities of daily living.

  • Functional Ability Evaluations
  • Physical Demands Analysis
  • Work Site Analysis
  • Activities of Daily Living Assessments
  • Attendant Care Analysis

Vocational Services:

National Health Partners vocational services consist of specific methods to determine an individual's employability and/or development of appropriate intervention to facilitate return to work. The vocational evaluation process is focused on the mental capacity for work and typically identifies the following:

  • Transferable skills
  • Vocational interests
  • Worker characteristics and behaviours
  • Academic abilities and potential
  • Learning styles
  • Work aptitudes and abilities
  • Suitable employment alternatives
In some cases, evaluation may also include assessment of an individual’s physical and/or psychosocial capacity for work.

National Health Partners utilizes Certified Vocational Evaluators and Vocational assessors for the delivery of our vocational services. Consultants are selected on their ability to match these requirements and receive further training and ongoing supervision by NHP in this field.
